Optimal resource allocation remains an enduring challenge for health systems globally. As healthcare expenditure escalates alongside demographic shifts and advancing medical technologies, health system administrators must balance technical efficiency, equity, and fiscal sustainability. Pareto efficiency—a foundational economic construct wherein no individual or sector can be made better off without making another worse off—provides a rigorous theoretical frame for evaluating health resource distribution. However, applying pure Pareto optimality to complex healthcare delivery presents unique ethical, structural, and political dilemmas.This paper establishes an operational framework for applying Pareto principles within modern healthcare management. We analyze three critical domains: (1) Exchange, production, and product-mix efficiency; (2) Strategic quality improvement tools (e.g., 80/20 root-cause mitigation and patient referral coordination); and (3) Cost-effectiveness and social welfare trade-offs (Kaldor-Hicks adjustments and distributive preferences). By integrating classic economic welfare theory with contemporary health system management and an illustrative hospital operational case, this research establishes a practical framework for leveraging Pareto optimization to eliminate systemic waste while safeguarding health equity.
